Physician Referral Form
If you would like to refer a patient for an office consultation, EGD or colonoscopy, please complete and submit this form. A member of our scheduling team will coordinate the specifics of the visit with you and your patient.
Fibroscan Referral Form
South Denver GI is the first GI practice in Denver to offer Fibroscan, a minimally invasive exam that measures the degree of scarring and fatty change in the liver. Please complete and submit this form to refer a patient for a Fibroscan examination.
