The Day of Your Procedure

When you arrive at one of our endoscopy centers for your procedure, please be sure to bring the following documents with you:

  • Medical history form (filled out within 30 days of your appointment)
  • Insurance card
  • Photo ID
  • A copy of your living will, advanced directive or medical power of attorney
